Online Application Procedures

For New Applicants:

  1. Click "Sign up".
  2. Fill out the online application form.
  3. Click "Submit" to review your entries.
  4. If you wish to edit your data, you may do so by clicking "Back". Otherwise, click on the "Submit" button to finally submit your application form. Once data is successfully submitted, it can no longer be altered.
  5. Take note of your reference number which will appear after application is successfully submitted.
  6. Print your application form.
  7. Then proceed to the UST Office for Admissions (OFAD) or at any Testing Center near you for payment and submission of the following requirements :

    1. Printed application form
    2. Clear photocopy of birth certificate
    3. Certificate of Candidate for Graduation to be signed by the School Principal/School Registrar
    4. Two (2) 2" x 2" recent identical pictures with printed name and signature at the back; and
    5. Payment of application fee

NOTES:

For Existing Applicants:

  1. Enter your reference number in the format given.
  2. Enter your surname.
  3. Click "Login" to view details of your application.
  4. If you wish to reprint your application form, you may do so by clicking on the "Print Form" button.

NOTE: No more changes can be made for existing applicants.
